Exemption Application to By-Law 60-2005 S 21-5

Introduction/Purpose
This application is to be completed by every person who requires a permit of exemption under section 21 (5) of the City of Burlington Animal Control By-law 60-2005 for prohibited, restricted and regulated animals. The City of Burlington may grant temporary exemptions for restricted or prohibited animals to be present for events held within city limits. Please submit this form and the required supporting documents to Burlington Animal Services a minimum of 1 week PRIOR to the event date. City staff will review your request; if approved, a permit outlining any conditions will be issued.
