Disclosure Request Form

Defendants or their representative may request disclosure. The disclosure is free of charge and contains all the information in the prosecutor's possession that is relevant to the charge. The disclosure request form must be completed in full for the request to be processed.  Refer to the Notice of Trial or Early Resolution Meeting Notice while completing the form. If you do not have the information that is required, you can contact the Court Administration Office.

Collection of Information
Personal information contained on this form is collected under the authority of the Provincial Offences Act, RSO, 1990, Chapter P.33. Section 46(2) and the Municipal Act, 2001, S.O. 2001, c. 25, as amended.  The information is used to process the request for disclosure and for further communication with the defendant or their representative.  This information will be shared with the Halton Court Services court administration that is administering the court matter.  Questions about the information collected can be directed to the Manager of Prosecutions, Halton Court Services 905-637-7790, ext. 6415.
