Conflict of Interest Form

Purpose:

When there is a real, perceived, or potential conflict of interest by any member of council, committee or local board the Municipal Conflict of Interest Act, requires the member to declare it.

Please complete the prescribed form to disclose a conflict/potential conflict of interest before the matter is considered at the meeting.
